If you choose to schedule a telehealth consultation with an ADVANCED TELE-GENETIC COUNSELING (AT-GC) genetic counselor, we require you to submit Personally Identifiable Information (i.e.: name, home address, telephone number, and email address) as well as Personal and Family Health Information. You are responsible for providing accurate Personally Identifiable Information as well as Personal and Family Health Information to ADVANCED TELE-GENETIC COUNSELING (AT-GC). Inaccuracies in any of the information you provide may directly affect the risk analysis, genetic test(s), and/or management information you receive through AT-GC’s telephone or video genetic counseling process and may affect our ability to contact you in the future. Importantly, you are responsible for keeping your preferred contact email address current, because this is how you will receive access to your Online Patient Portal and communication from AT-GC.

How Advanced Tele-Genetic Counseling (AT-GC) Protects Your Information

Storage

All user information is stored within HIPAA-compliant systems. Please refer to our Notice of Privacy Practices. You can ask us to change or delete your personal information by contacting us. You can also ask us to provide you a copy of any information we have about you and this will be done within 30 days.

Information provided online through our Website is protected using Secure Sockets Layer (SSL) Technology. Online communications from ADVANCED TELE-GENETIC COUNSELING (AT-GC) are performed through encrypted email or sent via an Online Patient Portal through our EMR system. Any information provided through our website should be done so at your own risk as we cannot warrant it will be fully secure.

 

Transmission

Electronic transmission of medical records and PHI may be performed by referring health care providers, the patient, and within our practice by authorized employees using one of our secure, HIPAA-compliant methods, including, but not limited to, secure fax, our EMR system and Online Patient Portal, cloud based file sharing, and encrypted email. Outside health care providers only have access to medical records of those patients they refer and will only receive consultation notes from a genetic counseling session from those patients.

 

Disclosure of Your Personal and/or Family Health Information

This Privacy Policy serves as notice that ADVANCED TELE-GENETIC COUNSELING (AT-GC) collects Personally Identifiable Information and Family Health Information in the manners described above. Except as set forth in this Privacy Policy or as specifically agreed by you, ADVANCED TELE-GENETIC COUNSELING (AT-GC) will not disclose any Personally Identifiable or Personal and/or Family Health Information it gathers from you on our Site. ADVANCED TELE-GENETIC COUNSELING (AT-GC) will only release Personally Identifiable or Personal and Family Health Information to third parties: a) to comply with valid legal requirements such as law, regulation, search warrant, subpoena, or court order; or b) in special cases such as a physical threat to you or others. All email communication coming from @at-gc.com uses an encryption service, but this does not mean that it does not have the possibility of being intercepted. ADVANCED TELE-GENETIC COUNSELING (AT-GC) will not be responsible for the security of any PHI sent to us in an unencrypted email. If you choose to send an unencrypted email rather than an encrypted email, you assume all responsibility for risks to your privacy and security of the information being sent. You are also solely responsible for the privacy and security of any information in your possession that may be stored in email, forwarded or sent to a third party.

This Privacy Policy does not fully protect you when you send Personally Identifiable Information, Personal and Family Health Information, other information, feedback, suggestions, business ideas, or concepts to ADVANCED TELE-GENETIC COUNSELING (AT-GC) by email. In order to keep this information strictly ‘private’, do not send it in an email message.

 

Appointment Reminders

ADVANCED TELE-GENETIC COUNSELING (AT-GC) appointment reminders may be sent through secure email from our online scheduler as well as through our EMR system and Online Patient Portal via text and/or email. You have the option to unsubscribe from these reminders in the email or text message.

 

Videoconferencing

ADVANCED TELE-GENETIC COUNSELING (AT-GC) uses a HIPAA/PIPEDA -compliant version of Zoom, called Zoom for Healthcare, to conduct telehealth genetic counseling appointments. You may refer to their Privacy Policy on their website.

 

Phone

ADVANCED TELE-GENETIC COUNSELING (AT-GC) uses an answering service which transmits HIPAA-compliant secure messaging to the appropriate contact. This is different than our voicemail system and PHI should not be left in a voicemail.

 

Contact Forms and Website Chat

ADVANCED TELE-GENETIC COUNSELING (AT-GC) will not be responsible for the security of any PHI sent to us using the website chat or online contact form found on our website. If you choose to send PHI over the chat or contact form, you assume all responsibility for risks to your privacy and security of the information being sent.

 

Payments

ADVANCED TELE-GENETIC COUNSELING (AT-GC) collects credit card or HSA/FSA payments for self-pay patients prior to or during the genetic counseling appointment. Credit card information and payments are collected through our online EMR system using Stripe. You may view the Privacy Policy on their website.  ADVANCED TELE-GENETIC COUNSELING (AT-GC) does not have access to your credit card information.

 

Security Measures

ADVANCED TELE-GENETIC COUNSELING (AT-GC) requires users to set a password to the Online Patient Portal using our EMR system in order for users to access their Personally Identifiable Information or Personal and/or Family Health Information.

ADVANCED TELE-GENETIC COUNSELING (AT-GC) utilizes Secure Socket Layer (SSL) encryption in transmitting Personally Identifiable Information.

ADVANCED TELE-GENETIC COUNSELING (AT-GC) closely monitors the limited number of employees and contractors who could have access to your Personally Identifiable Information.

ADVANCED TELE-GENETIC COUNSELING (AT-GC) requires all AT-GC employees and contractors to abide by our Privacy Policy. All AT-GC employees and contractors maintain appropriate training in compliance to HIPPA.

ADVANCED TELE-GENETIC COUNSELING (AT-GC) regularly maintains and backs up its systems to protect the integrity of your Personally Identifiable and Personal and/or Family Health Information.

Despite our efforts to protect your Personally Identifiable Information and Personal and/or Family Health Information, there is always some risk that an unauthorized third party may find a way around our security systems or that transmission of your information over the Internet will be intercepted.
